We’re looking for an experienced Social Worker to join our leadership team in the Post-Pathway Discharge Service on a part-time basis (16 hours per week). The Post-Pathway Discharge Team supports people to leave hospital safely and smoothly, ensuring they get the right care and support once they return home or move to a community setting. They work closely with hospitals, community health services, and social care teams to coordinate care, provide expert advice on complex cases, and help improve discharge processes so people can recover in the right place, at the right time.

 

What you’ll be doing:

 

  • Supporting and managing the hospital discharge pathway service
  • Providing advice on complex cases
  • Helping the team deliver the best possible outcomes for patients
  • Driving service improvement and efficiency

 

You’ll be great for this if you:

 

  • Are a qualified Social Worker with leadership experience
