You will be conducting comprehensive assessments of older individuals' mental health needs, developing and implementing care plans to support their mental well-being, providing ongoing individualised support and interventions, and collaborate with multidisciplinary teams to ensure a holistic approach to care.

To be considered for this position, it's essential to hold a Social Work Qualification and maintain registration with SWE.
