Your Role:
 

  • Conduct timely assessments of patients in hospital to plan safe and effective discharges.
  • Work closely with multi-disciplinary teams to coordinate care and support for patients transitioning from hospital to home or other care settings.
  • Provide immediate guidance, support, and interventions to ensure smooth discharges while maintaining patient safety and well-being.
  • Collaborate with external agencies, care providers, and community services to meet patient needs.
  • Use your strong interpersonal skills, empathy, and commitment to patient-centered care to make a real impact.
  • Help reduce hospital stays and support individuals to regain independence in their own homes whenever possible.

What We Need:
 
  • You must hold a Social Work Qualification and be registered with SWE.
  • Previous experience in hospital social work or discharge planning is highly desirable.
