This is a full time, permanent role working as a Nurse Case Manager (complex care), in the Merseyside and Cheshire region. The role is to work Monday to Friday, 9am – 5pm, with no nights, weekends, or bank holiday working. The salary is £43,000 per annum. You must be a driver with car for this community role.
What You’ll Do As a Community Registered Nurse, you’ll play a key role in supporting and overseeing high-quality care for individuals with complex health needs, including those with:
Spinal or neurological injuries
Acquired brain injuries or trauma
Epilepsy, learning disabilities, or genetic disorders
Your responsibilities will include:
Assessing, reviewing, and maintaining bespoke care packages
Providing hands-on clinical training and supervision to care staff
Offering expert guidance and support to clients and their families
Collaborating with health and social care partners (ICBs, hospitals, case managers, and other professionals)
Participating in a paid on-call rota on a rotational basis

A Registered Nurse (Adult or Paediatric) with current NMC registration
Experienced in ICU, HDU, or complex care environments
A confident communicator and natural leader
