We are currently recruiting a Care Coordinator to support children and adults with complex physical disabilities and life-limiting conditions across West London. This is a fantastic opportunity to join a compassionate, nurse-led team delivering high-quality care in clients’ homes.

What you’ll do as a Care Coordinator:

• Coordinate and oversee the mobilisation of new complex care packages

• Set up care plans, complete risk assessments, and ensure regulatory compliance

• Liaise with clients, families, healthcare professionals, and internal teams

• Ensure timely and seamless delivery of care services

• Maintain strong working relationships across the organisation

What we’re looking for:

• Previous experience in coordinating care packages (complex care preferred)

• Knowledge of health and social care legislation

• Flexible, proactive approach to problem-solving

• Confident in handling data and making informed decisions
