We are currently recruiting experienced Hospital Discharge Social Workers to join a dynamic and fast-paced multidisciplinary team. You will work closely with NHS colleagues, ward staff, therapists, and other professionals to support safe and timely discharges from hospital settings.

Your responsibilities will include undertaking Care Act assessments, coordinating discharge planning, managing complex cases, and ensuring that appropriate care and support packages are in place to promote safe recovery and independence in the community.

Applicants must hold a recognised Social Work qualification and be registered with Social Work England (SWE). Previous experience in hospital discharge, safeguarding work, and integrated health and social care settings is essential. Experience using Mosaic or similar case management systems is beneficial.
