Your main duties will include:
 

• Supporting and advocating for adults with complex health and care needs through the Continuing Healthcare (CHC) pathway.
• Working collaboratively with multidisciplinary professionals, service users, and families to assess needs and develop effective care plans.
• Contributing to CHC assessments, reviews, and decision-making processes to ensure individuals receive appropriate funding and support.
• Promoting independence, dignity, and person-centred care for adults with long-term health conditions and complex needs.

• A comprehensive wellbeing package supporting your physical, mental, and emotional wellbeing – available to both you and your family.
• 6 days of protected time each year dedicated to maintaining your Continuous Professional Development (CPD).
• A relocation package of up to £8,000 (subject to eligibility).
