The Role

As Nurse Case Manager, you’ll take ownership of complex care packages for adult clients, ensuring safe, high-quality delivery while developing and supporting care teams.

Key responsibilities:

• Managing and reviewing complex care packages in the community
• Leading clinical quality, governance, and best practice
• Delivering specialist training (e.g. tracheostomy, ventilation)
• Assessing and signing off staff competencies
• Coaching and mentoring care teams
• Building strong relationships with clients, families, and MDTs

About You

• Registered Nurse with NMC PIN
• Experience in complex care (e.g. tracheostomy, ventilation, spinal injury)
• Passion for developing others and improving care standards
• Strong communication and organisational skills
• Confident working independently in a community setting
• Full UK driving licence and access to a vehicle
