We’re recruiting for an exciting locum opportunity paying £36 per hour within a fast-paced Transfer of Care and Hospital Discharge Team. This is a full-time office-based role, ideal for someone who enjoys being part of a close-knit team, having easy access to day-to-day support and working collaboratively with healthcare professionals.

The role

 

You’ll work within a dynamic hospital discharge service, supporting people across a 200-bed pathway to return home safely with the right package of care in place. The service provides an effective route for re-referral where longer-term support is needed.

 

You’ll complete assessments within 14 to 28 days, carry out six to 12-week reviews and contribute to a light-touch triage process. Working closely with healthcare colleagues, you’ll help ensure smooth transitions from hospital to home, promote independence and reduce avoidable readmissions.

 

What you’ll need

 

• Social Work qualification
• Current registration with Social Work England
• Experience within adult social care, hospital discharge or Transfer of Care services
• Strong assessment, care planning and review skills
• Confidence working in a fast-paced, multidisciplinary environment
• A person-centred approach and commitment to achieving positive outcomes
