Key Responsibilities

• Support medicines optimisation through medication review, reconciliation and prescription review.
• Provide expert medicines advice to patients, carers, consultants and staff.
• Assist with safe procurement, management and control of medication stock.
• Contribute to clinical governance, audits, incident reporting and the introduction of new medicines and therapies.

About You

• You are a registered Pharmacist with the GPhC.
• You have hospital pharmacy experience and a clinical diploma or equivalent experience.
• You are an independent prescriber or have experience in secondary care pharmacy.
