The Role
As Care Coordinator, you’ll play a central role in ensuring care packages run smoothly, safely, and efficiently. You’ll act as the link between clients, families, and care staff—making sure the right support is in place at the right time.
This is a fast-paced, rewarding position suited to someone who thrives on organisation, communication, and making a difference.

Key Responsibilities
  • Act as the main point of contact for clients and their families
  • Coordinate care plans and build effective staff rotas
  • Manage staff availability and ensure care packages are fully covered
  • Liaise with care teams to maintain continuity and quality of care
  • Process and verify timesheets for payroll
  • Support recruitment, onboarding, and ongoing staff performance
  • Conduct quality monitoring visits
  • Maintain accurate digital and paper records
  • Participate in the on-call rota, including out-of-hours support

About You
  • Experience in a care coordination, scheduling, or similar role (desirable)
  • Strong organisational and time management skills
  • Comfortable working in a fast-paced, people-focused environment
  • Proactive, solution-oriented approach
  • Excellent communication skills
  • Full UK driving licence with access to a vehicle and appropriate insurance
This role would suit someone who has worked as a Care Planner, Care Coordinator, Care Administrator, or Scheduling Coordinator.
