A supportive Local Authority team in North West London is looking for an ambitious and compassionate Adult Front Door Social Worker to join its Adult Social Care service. Offering a salary of £44,000–£52,000, this is an excellent opportunity to work in a fast-paced, rewarding environment, supporting adults to access the right help at the right time.

As part of the Front Door team, you will be at the heart of Adult Social Care - responding to referrals, assessing needs, managing risk and helping people retain independence wherever possible.

The role

You will play a key role in ensuring adults and their families receive a timely, strengths-based response. Your responsibilities will include:
 

• Receiving, screening and prioritising new Adult Social Care referrals
• Completing Care Act assessments, reviews and proportionate safeguarding enquiries
• Assessing risk and making informed decisions in often complex situations
• Supporting adults, carers and families to identify practical outcomes and appropriate support
• Signposting people to community, voluntary-sector and preventative services where appropriate
• Working closely with health colleagues, safeguarding professionals and other partner agencies
• Managing a varied workload while maintaining accurate, high-quality case records
• Contributing to safe, effective and person-centred Adult Social Care practice

• £44,000–£52,000 per annum, dependent on experience

• Strong written communication, assessment and decision-making skills
