Are you an experienced Adult Nurse looking to develop your career within community complex care? This Adult Clinical Lead opportunity offers the chance to take clinical responsibility for complex care packages across South London and surrounding areas, combining office-based working with home working and time in the community.
You will work for an established community complex care provider, supporting adults with a range of complex healthcare needs to live safely and independently within their own homes. The position offers excellent clinical autonomy, varied community-based work and the opportunity to develop your clinical and management experience.

The Role
As Adult Clinical Lead, you will provide clinical oversight of complex community care packages, completing assessments and reviews while ensuring care teams have the skills, training and competencies required to deliver safe and effective care.
You will manage your own diary and work across the South London office, service users' homes and community settings, with some flexibility to work from home.
Key Responsibilities
• Complete comprehensive clinical assessments and ongoing reviews of complex care packages.
• Develop and review person-centred care plans and risk assessments.
• Provide clinical oversight and support for adults receiving complex care at home.
• Deliver practical clinical training, supervision and competency assessments for care workers.
• Support the mobilisation and ongoing management of care packages.
• Work closely with ICBs, Case Managers, Social Services, NHS and private hospitals and other healthcare professionals.
• Provide clinical guidance to care teams at the point of care.
• Maintain high standards of safeguarding, clinical governance and risk management.
• Participate in a paid rotational telephone on-call service.
About You
To be considered for this Adult Clinical Lead position, you will need:
• Registered Adult Nurse qualification with an active NMC PIN.
• Previous adult complex care or community nursing experience.
• Strong clinical experience supporting complex healthcare needs.
• Tracheostomy and ventilation experience.
• Experience completing clinical assessments, care plans and risk assessments.
• Confidence delivering clinical training, supervision and competency assessments.
• Strong safeguarding and clinical governance knowledge.
• Excellent communication and multi-agency working skills.
• Full UK driving licence.
• Comfortable travelling across South London, Surrey and surrounding areas.
