Complex Care Case Manager – Merseyside
Up to £35,000 + Bonus + On-Call Payments | Monday–Friday | Full Time | Permanent

Are you an experienced Complex Care Coordinator, Care Manager or Field Care Supervisor looking to take the next step in your career?

We're recruiting for a Complex Care Case Manager to join an established specialist care provider, overseeing community-based care packages across Merseyside.

This is a fantastic opportunity to take ownership of your own caseload, build strong relationships with clients and families, and lead dedicated care teams delivering high-quality, personalised support in people's own homes.

Your Role

As a Complex Care Case Manager, you'll oversee the delivery of high-quality care packages for individuals with complex healthcare needs across Merseyside.

Your responsibilities will include:

  • Managing a caseload of community-based complex care packages
  • Completing care assessments, care plans and risk assessments
  • Coordinating and mobilising new care packages
  • Leading, supervising and developing community care teams
  • Building strong relationships with clients, families and healthcare professionals
  • Ensuring compliance with CQC standards and safeguarding requirements
  • Monitoring care quality and driving continuous improvement

About You

I'm keen to hear from experienced Care Managers, Complex Care Coordinators, Case Managers and Field Care Supervisors who are confident managing care packages in the community.

You'll ideally have:

  • Experience coordinating or managing complex care packages
  • Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs
  • Experience supervising and supporting care teams
  • Good understanding of CQC regulations and safeguarding
  • Strong organisational, communication and leadership skills
  • Full UK driving licence and access to your own vehicle
