Package & Benefits
- Salary £45,000 per annum.

Key Responsibilities
- Lead care delivery and maintain high clinical standards.
- Manage and review bespoke care packages for adults with complex needs.
- Complete clinical assessments, care planning, and medication management.
- Lead, train, and support carers delivering care in the community.
About You
- Must be a Registered General Nurse with a valid NMC PIN.
- Recent hands-on experience with tracheostomy care and ventilated patients.
- Experience working at Band 5 or Band 6 level in community or complex care.
