Join a well-established provider of complex care packages as a Paediatric Case Manager! This role will be covering care packages commissioned by Buckinghamshire, Oxford and West Berkshire ICB.
The successful Paediatric Nurse Manager will be responsible for the overall provision of complex care, whilst creating personalised care plans for each service user. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.
To work as a Paediatric Nurse Manager, you must...
- Be an RSCN with a minimum of 2 years’ experience in the UK with an active NMC PIN
- Possess a supportive management style that can bring out the best in others
- Have up to date experience of tracheostomy care and ventilators
- Have advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
- A full UK Driving Licence is essential
The key responsibilities include...
- Designing and delivering a tailored care plan specific to an individual clients needs
- Effectively managing the MDT through strong communication and teamwork
- Providing ongoing monitoring, support and assessment of clients physical and physiological needs within your caseload
- Build and maintain sophisticated relationships with your clients and family members
- Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care
