Your primary duty will involve working closely with clients to develop tailored care plans that address their emotional, social, and psychological concerns. You will provide counseling, therapy, and support services to assist seniors in coping with life transitions, grief, loneliness, and other mental health issues. Additionally, you will play a pivotal role in connecting clients to relevant resources, such as support groups, community programs, or specialized healthcare services, all while advocating for their rights and needs within the healthcare system.
