Nurse Case Manager

As a Community Nurse Case Manager, you will:

• Conduct initial assessments in the community.
• Train and assess workers' competencies and allocate support workers to patients.
• Collaborate with the wider multidisciplinary team (MDT).
• Attend meetings and make referrals.
• Hold team meetings.
• Manage a team of non-clinical case managers.

• Annual salary of £43,000.

• Full-time hours: 37.5 hours per week, Monday to Friday.

The ideal Community Nurse Case Manager will:

• Be an RGN or RSCN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management.
• Have experience in tracheostomy and complex care.
• Be a car driver, flexible to travel around the North West (Manchester, Chester, and the Wirral).
• Have experience caring for both adults and children.
