Nurse Case Manager

Are you a dedicated RGN Qualified Nurse with a passion for complex care? Our client is seeking a Community Nurse Case Manager to support clients in Birmingham / Midlands region.  This role offers a unique blend of home-based work and community visits, ensuring clients can live independently at home and within their wider communities.
This role offers a competitive salary of £48,000 plus the opportunity to earn a bonus. You'll enjoy the flexibility of working from home two days a week, with the remaining three days spent visiting clients in the community. Additionally, you'll have the chance to make a real difference in the lives of those with complex care needs.

As a Community Nurse Case Manager, your responsibilities will include:

• Conducting initial assessments in the community
• Providing clinical oversight to your team
• Collaborating with the wider multidisciplinary team (MDT)
• Attending meetings and making necessary referrals
• Managing a team of non-clinical case managers

• Annual salary of £48,000
• Bonus opportunities
• Full-time, permanent position
• Monday to Friday, 9am-5pm with some flexibility
• Two days home-based, three days community visits
• Travel across Birmingham and surrounding areas

• RGN qualification with excellent clinical skills in complex care
• Experience with peg feeds, ventilators, and tracheostomy management
• A valid driving licence, as the role involves remote and community-based work
