As a Dementia Nurse Practitioner, you will:

• Support, deliver, and coordinate person-centred dementia care.
• Implement therapeutic interventions to improve clinical outcomes.
• Work within a multidisciplinary team of nursing, social care, psychology, and wellbeing practitioners.
• Conduct holistic nursing assessments.
• Provide general nursing and bio-psychosocial interventions.
• Share knowledge and deliver best practices.
• Use critical reflective practice.
• Operate as an autonomous practitioner.

• An active NMC Pin.
• Minimum of 2 years' practice experience in a dementia care community.
• Evidence of delivering person-centred care for people with complex dementia needs.
• Experience in collaborative and multidisciplinary team working.
• Strong communication skills, flexibility, and creativity.
• Commitment to quality care and professional development.
