As a Community Nurse Case Manager, you will:

  • Manage a caseload of clients with complex needs in the community.
  • Conduct initial assessments in the community.
  • Train and assess workers' competencies and allocate support workers to patients.
  • Collaborate with the wider MDT.
  • Attend meetings and make referrals.
  • Hold team meetings.
  • Manage a team of non-clinical case managers.

  • Have experience with tracheostomy and ventilator care.
