Community Nurse Case Manager

As a Community Nurse Case Manager, you will:

• Manage a caseload of clients with complex needs in the community.
• Conduct initial assessments in the community.
• Train and assess workers' competencies and allocate support workers to patients.
• Collaborate with the wider MDT.
• Attend meetings and make referrals.
• Hold team meetings.
• Manage a team of non-clinical case managers.

The ideal candidate for the Community Nurse Case Manager role will:

• Be an RGN Qualified Nurse.
• Have excellent clinical skills in complex care, including PEG feeds, ventilators, and tracheostomy management.
• Be a car driver, as the role is community-based.
• Have experience with tracheostomy and ventilator care.
